Passport applications

Let the Library help you get where you want to go with our passport processing service! Open to U.S. citizens who have never before had a passport or who are ineligible to renew, we can process your passport application.

Make an appointment

Appointments are required for passport applications and are available weekdays, evenings and weekends, up to 30 days in advance. Next-day appointments may also be available. All appointments are offered at Old Worthington Library, in downtown Worthington on High Street.

To make an appointment, we will need your name, email address and phone number and whether you need passport photos. Applicants under 16 will also need to provide a parent or guardian's name.

Need more than one passport? Please make a separate appointment for each applicant.

Preparing for your appointment

You will need to bring the following to your appointment:

  1. An original copy of proof of U.S. citizenship (one of the following):
    • U.S. birth certificate, Consular Report of Birth Abroad or Certification of Birth
    • Certificate of Naturalization
    • Certificate of Citizenship
  2. Current Ohio driver's license, military ID or government ID (for applicants 16 years of age or older)
  3. Completed application form DS-11:
  4. One recent (not older than 6 months), color, 2"x2" photo of applicant, following the U.S. Department of State's photo requirements. The library's certified passport agents can take a photo, if preferred (for applicants 3 years of age or older).
  5. Fees:
    • passport book (for all international travel): $110, adult; $80, minor
      only checks or money orders made payable to U.S. Department of State will be accepted
    • passport card (only for land and sea travel between the U.S., Canada, Mexico, Bermuda and the Caribbean): $30, adult; $15, minor
      only checks or money orders made payable to U.S. Department of State will be accepted
    • processing fee: $25
      cash, credit cards or checks or money orders made payable to Worthington Libraries will be accepted
    • photo fee (optional): $10 per photo
      cash, credit cards or checks or money orders made payable to Worthington Libraries will be accepted
    • expedited service fee (optional, to receive passport in six weeks or less): $60
      only checks or money orders made payable to U.S. Department of State will be accepted

Applicants 15 years of age or under

Additional documentation is required for applicants under the age of 16. Both parents/guardians must authorize the issuance of the child's passport. The best way to do this is for both parents/guardians to go with the child in person to apply for the passport. If this is not possible, please ask a library staff member for help.

  1. Documentation that lists the parent(s) or legal guardian(s) of the child applying for a passport (one of the following):
    • U.S. birth certificate, Consular Report of Birth Abroad (also evidence of U.S. citizenship)
    • foreign birth certificate
    • adoption decree
    • divorce/custody decree
  2. Each parent/guardian must present a current Ohio driver's license, military ID or government ID
  3. Additional photocopies of BOTH the proof of U.S. citizenship (listed above) and parent/guardian's current Ohio driver's license, military ID or government ID
