Changes from the IRS in recent years mean we will not have the large selection of tax forms you've come to expect in the past.
But we've got you covered! In addition to copies of Form 1040, Form 1040A and Form 1040EZ (and their corresponding instruction booklets), we have several ways to help you through the 2017 tax season.
While library staff members cannot prepare taxes or answer specific tax-related questions, they can assist you in locating the information you need. Library staff is available to help locate forms and identify additional tax resources.

Due to budget cuts, the IRS will be providing a reduced number of tax forms, instruction booklets, publications, schedules and worksheets to libraries. Reproducible copies of all forms and instructions will be available at library information desks. Federal tax forms can be ordered to be delivered by mail at IRS.gov/orderforms, or by calling 1-800-829-3676 .
The Ohio Department of Taxation has consolidated forms IT 1040, IT 1040 EZ and amended IT 1040X into one Universal IT 1040 tax return. Ohio tax forms can be downloaded at tax.ohio.gov/Forms or requested to be delivered by mail by calling 1-800-282-1782.
