Let the Library help you get where you want to go with our passport processing service! Open to U.S. citizens who have never before had a passport or who are ineligible to renew, we can process your passport application.

Passport renewal

Passport expired? In most circumstances, you can renew by mail directly with the U.S. Department of State.

Passport types

You can apply for a passport book, which is valid for all international travel. You can also apply for a less expensive passport card, which is valid only for land and sea travel between the U.S., Canada, Mexico, Bermuda and the Caribbean.

Passport photos

The U.S. Department of State has specific requirements for passport photos. Library passport agents do not take passport photos, but several local stores offer passport photos, including including CVS, FedEx Office, Staples, UPS Store and Walgreens.

New passport application

Appointments are required for passport applications and are available weekdays, evenings and weekends, up to 30 days in advance. Next-day appointments may also be available. All appointments are offered at Old Worthington Library, in downtown Worthington on High Street.

To make an appointment, we will need the names and ages of each applicant. Applicants under 16 will also need to provide a parent or guardian's name. We require an email address and phone number to confirm your appointment.

We can accommodate up to two passport applications with a single appointment. Need more than two passports? Please make a separate appointment for additional applicants.

Preparing for your appointment

You will need to bring the following to your appointment:

1. An original copy of proof of U.S. citizenship (one of the following):
   • U.S. birth certificate, Consular Report of Birth Abroad or Certification of Birth
   • Certificate of Naturalization
   • Certificate of Citizenship
   • Fully-valid, undamaged U.S. passport (may be expired)
2. Current Ohio driver's license, military ID or government ID (for applicants 16 years of age or older)
3. Completed application form DS-11:
   • fill out online and print
   • download and print [PDF] to fill out by hand
   • pick one up at Old Worthington Library (please allow extra time to fill out before your appointment)
4. One recent (not older than 6 months), color, 2"x2" photo of applicant, following the U.S. Department of State's photo requirements.
5. Fees in the correct form of payment:
   • Some fees must be paid directly to the U.S. Department of State and others to Worthington Libraries. 
   • The U.S. Department of State accepts only checks or money orders. Worthington Libraries can accept cash and credit cards, as well as checks or money orders.
   • To pay all fees by check or money order, you must bring two separate checks or money orders, one payable to the U.S. Department of State and one to Worthington Libraries. Payments cannot be combined in a single check or money order.

Applicants 15 years of age or under

Additional documentation is required for applicants under the age of 16. Both parents/guardians must authorize the issuance of the child's passport. The best way to do this is for both parents/guardians to go with the child in person to apply for the passport.

6. Documentation that lists the parent(s) or legal guardian(s) of the child applying for a passport (one of the following):
   • U.S. birth certificate, Consular Report of Birth Abroad (also evidence of U.S. citizenship)
   • foreign birth certificate
   • adoption decree
   • divorce/custody decree
7. Each parent/guardian must present a current Ohio driver's license, military ID or government ID
8. Additional photocopies of BOTH the proof of U.S. citizenship (listed above) and parent/guardian's current Ohio driver's license, military ID or government ID
